Be A Donor: 5 Simple Steps

It is important that your decision be properly recorded and communicated.

1. Get started

The link above allows you to download a simple form that you can make part of your will, give to your family and friends.

We make this section public because it helps us to be transparent in our education of whole body donations.

Here is what the forms say in plain language:

2. What the form means

You are filling out a bequest which means that by signing this, you intend to donate your body to education, science or research purposes.

3. Witnesses

You will need to find at least one witness and preferably two. Part of LifeDonation's mission is to demystify the donation process and that includes proper understanding on the part of the immediate loved ones and friends.

4. LifeDonation.org works with a community of research and educational institutions

Because it is hoped that you will live a prosperous life following your bequest, no one can predict whether or not certain institutions are still available to accept your body years from now.

Therefore, there are rules in place that make it inappropriate for any institution to guarantee it will accept your body.

LifeDonation is available to discuss individually all options for anyone who has a donation preference.

Finally,

You will need to send a signed copy of your bequest to LifeDonation and keep a copy with your final will. Once we receive your signed bequest, we will send you a permanent donor card for your wallet or purse.
